We hope your first visit to our practice will lead to a

relationship of many years where you develop a strong

bond with your GP , nursing and receptionist

staff who care for you and your family. Our practice

serves the local community of Campbellfield, keeping

you healthy by appropriate medical treatment when

needed and sharing advice on how to maintain good

health and prevent illness whenever possible.

Location

Located at 1655 Sydney Road Campbellfield, in the

heart of Northern suburbs of Melbourne, our clinic is

close to pharmacy and radiology services allowing the

convenience of healthcare, with 4CYTE Pathology

occupying one room.

Parking

We have six parking spaces available on-site,

including a disabled space. There is further on street

parking available along Augusta Avenue.

Walk-in Policy

We accept walk-ins however they may need to wait whilst booked patients are seen first.

Fees and Billing Arrangements

We are a mixed-billing clinic.

Fees are payable at the time of consultation by cash or

credit/debit cards.

The current fees can be seen at reception or on our

website for further details.

Children under 16, valid Pension, valid DVA & valid HCC

holders, individuals aged above 65, Refugees and ATSI

will be bulk billed.

All accounts must be settled on the day. We have a no

credit policy. If you have any questions regarding our

fees, please discuss this with us.

Cancellation Policy

Please provide at least 2 hours’ notice if you are unable

to attend the appointment. Failure to do so will incur a

$50 cancelation fee. Repeated no shows without

notice may result in a restriction being placed on your

patient record, and you will not be able to make further

appointments.

Appointments

Please ring (03) 8577 8060 to make an appointment or

book online at www.newleafmedical.com.au. Every

effort will be made to accommodate your preferred

time. Emergencies will always be given priority and our

reception staff will attempt to contact you if there is

any unforeseen delay or your GP has been called away.

Longer consultation times are available, so please ask

our receptionists if you require some extra time, for

example when consulting for multiple problems or

completing forms. Please let us know this when you

book the appointment.

New Patients

It is a good idea to arrive at least 10 minutes early for

your first appointment if you have not completed your

registration online. Please ensure you bring the

following items to your first appointment:

• A current Medicare Card

• Private health Fund Details

• Concession cards include Health Care Card,

Eligible Veterans Affairs (DVA) card or pension

card.

• Any relevant test results or referrals from

another clinic or specialist

Change of details

If any of your personal details change or require

updating, we request you please inform reception via

phone or at your next appointment. This can include

change of name, address, contact number, email

address or next of kin.

Face Masks

Patients attending New Leaf Medical clinic for an in-

clinic consult with respiratory symptoms are required

to wear a face mask upon entering the clinic.

After-hours Emergency

If you need to see a doctor while the clinic is closed,

please call our after-hours service provider National

Home Doctor Service on 137425. If it’s an emergency,

please dial 000.

Your rights, feedback, and complaints

If you have a problem, we would like to hear about it.

Please feel free to talk to your doctor or the

receptionist. You may prefer to write to us or use our

suggestion box. We take your concerns, suggestions,

and complaints seriously. Should you need to make

If you feel there is a need to take your concern further,

you may prefer to contact the Health Services

Commissioner on 1300 582 113.

Reminder System

Our practice is committed to preventative care. We will

automatically include you on our reminder system. We

may issue you with a reminder notice from time to time

offering you preventative health services appropriate

to your care. If you do not wish to be part of this system,

please let your doctor or receptionist know.

Results Policy

In the interests of best practice and privacy, and unless

otherwise advised, you should book an appointment

72 hours after any investigations/tests have been

performed. It is your responsibility to book this follow up

appointment to get your results.

Home and other visits

Home visits are available at the discretion of our GPs.

Due to a high workload, this is not always available and

for matters of medical urgency or illnesses that require

hospital assessment or care, you may be asked to

attend the local hospital.

After-hours Emergency

Home Doctor Service on 137425. If it’s an emergency,

please dial 000.

Telephone access

If you phone the Clinic requesting to speak to your

doctor, you will be asked to leave a number on which

you can be contacted. We aim to return phone calls

within 24 hours, alternatively you may book an appointment via Telehealth if non urgent.

Should you feel seriously unwell, be suffering chest

pain or severe shortness of breath, you should call an

ambulance on 000 immediately. Minutes could save

your life.

Privacy Information

Your medical record is a confidential document. It is

the policy of this practice to maintain the security of

personal health information at all times and to ensure

that this information is only available to authorized

members of staff. We abide by the national Privacy

Principles available here:

www.privacy.gov.au/health/index.html. Copies of the

Practice Privacy Policy are available at reception.

Communication

In line with our privacy policy, our practice will only

engage in communicate via telephone, SMS message,

in person and fax. We are not able to guarantee the

security of emails, sent or received. EMAILS WILL NOT BE

USED to discuss or diagnose any medical issues or to

book appointments as it is intended to be primarily

used for one-way communication to patients.
